Hello!

Welcome on - Ask a Doctor - service!

Regarding your concern, I would explain that considering you medical history, the first opinion would be in favour of heart failure symptomatology.

Anyway, in case no exacerbating factors are noted (like cardiac arrhythmia, a pulmonary infection, or excessive blood pressure fluctuations, etc.) then other alternatives should be sought (like pulmonary embolism considering your recent surgery).

Coming to this point, I would recommended discussing with your attending internist or cardiologist on the possibility of performing a physical exam and cardiac ultrasound for evaluating the right heary and potential pulmonary pressure.
If necessary further tests would be recommended.

Hope to have been helpful to you!

In case of any further questions, feel free to ask me again.

Kind regards,
