Question: Hi,
I had surgery on Feb 6th 2020 to remove external and internal hemorrhoids, i am still bleeding with yellowish discharge. Its bright red blood when i have a stool movement around two tablespoons. I have to keep checking and wiping every couple of hours to make sure that blood isnt in my underwear. Also, i smell and have to shower or bath atleast twice to three times daily. My doctor gave me rectiv to use for a couple of months, i was soaking for 15 minutes 3 times a day. His next step was magnesium sulfate for a couple of weeks,... I dont have any health issues exceot asthma ( only when needed) which is durning pollen season, spring.

Has the doctor examined you proctoscopically post surgery,
Recurrence of Hemorrhoids post surgery is very common and you had both internal and external hemorrhoid(so its even more common in your case).
Please write if you have been examined by the treating doctor to check for recurrence?
And which surgery was performed Staplectomy done?

Follow up: Dr. Ramesh Kumar (8 hours later)
underwent 3 quadrant closed hemorrhoidectomy, colonoscopy and rubber band ligation of internal hemorrhoids in February 2020. His follow-up waas delayed due to COVID-19 restrictions. He presents now with history of fecal soiling, occasional blood on the toilet paper and 1 out of 10 anal pain. Examination today on inspection there is granulation tissue in the right lateral quadrant c/w hemorrhoidectomy incision. Digital rectal examination was performed revealing good sphincter tone and no masses. Anoscopy in one quadrant was performed revealing granulation tissue in the right lateral quadrant with was treated with silver nitrate. 2% lidocaine jelly was applied after the application of silver nitrate. Four quadrant anoscopy was not performed due to patient discomfort and will need to be perform on the next visit. Impression: Status post closed hemorrhoidectomy complicated by granulation tissue in the right lateral quadrant causing the above-mentioned symptoms.Recommendations: Continue lidocaine jelly for pain relief. Tub baths 2-3 times a day. Follow-up in 2 weeks to assess whether additional treatment with silver nitrate warranted. Diseased process was discussed at length with the patient.

u 02/6/2020: OPERATIVE PROCEDURE: 1. Total colonoscopy. 2. Three quadrant closed hemorrhoidectomy. 3. Rubber banding of internal hemorrhoids.Grade 3 prolapsing internal and external hemorrhoids.Pt. hx. Grade 3 prolapsing internal and external hemorrhoids. Rectal bleeding. Currently the patient expressed pain 1/10 today: rectal site. Usingmedications: Lidocaine and Nitroglycerin creams as ordered. Presenting red stains in stool like size of a quarter prn around every 3 days. He does refer daily yellow colored discharge from anal site. Aprox. 1 BM daily light brown colored. He is taking protein shakes, eating abundant vegetables and moderate amount of fruits and using Metamucil 1tbls 3 times a day. Ingests around 8-10 glasses of water daily. Performing daily cardiovascular and strength weight exercises daily. MD notified.

He recently had hemorrhoidectomy. Doing OK - still having problems - having a foul smelling yellow mucus discharge. He does have rectal pain which is worse with bowel movements or if he tries to exercise

Hello again sir,
Formation of granulation tissue means that the scars are healing whenever the stool passes from that place it stretches the healing scar causing pain (same theory is with exercise).
As the mucosa layer is stretched it starts to secrete mucous you could see in stools.
Its all part and parcel of healing.
Dont worry,
Give body rest till it heals completely.

I have rested for 1 year and 2 months, i was told no longer than a couple of weeks. How long does it take for the bleeding and drainage to stop.

I only exercise, no strenuous activity, i soak, i drink metamucil, eat healthy

If that is the case then I would suggest you to get a MR scan of pelvis done to rule out any tissue level infection.
