I Fear If There Could Be Something Wrong With The Nevus Or If Could Semi Big Nevus Inflames Could Be Dangerous?

Question: Hello

I've tried and searched for 5 months now for an idea of what is wrong with my leg.

I have 3 semi big nevus inflameus on my body, 2 on my arm and 1 on my leg and many tiny ones around. They are warm to the touch and 2 of them are visible red and the one on my leg is hard to see in some lights, perhaps a pink/skin colour version.

My problem is that I started noticing a swelling right under my nevus on my leg, it's placed on top of my shin bone and halfway between the ankle and the knee. It's around 3 cm long and 2 cm wide and I've always had it. I think there has always been something under the birthmark that I could feel, maybe a collection of veins or something? It doesn't feel like water under it feels more normal and I can move it around with the skin. Problem is that I started getting sore and tired in the area after I changed job and had many hours on my legs and I thought maybe I had gotten a shin splint and started noticing what appeared like a swelling right under the nevus.

Again I think there has always been something like a swelling under it, can that be true and what could be under a nevus that I can feel and see?

My biggest worry is that I fear if there could be something wrong with the nevus or if this could be dangerous? I've been at the doctor but they can't really see the birthmark and they said it sounded like a shin splint, but this is 3 months ago. I feel this sore feeling in the leg when I lift my foot up while walking fast sometimes and some days it's worse than others even though I sit most of the day now.

Any advice on this would be greatly apreaciated and also more information if I should worry about Nevus Flameus type of birthmarks or if they are totally benign?

I am Dr Kakkar (Dermatologist and Venereologist). I have noted your concern.
Vascular malformations can be either capillary, venous, lymphatic or they can be mixed. Nevus Flameus is a capillary vascular malformations and it can be associated with other deeper vascular malformations like venous or lymphatic. This swelling could be one of these malformations or it could just be a soft tissue hypertrophy secondary to Nevus Flameus. The exact nature of it can only be known by either a MRI scan or may be a high frequency ultrasound. Nevus Flameus on leg is mostly a cosmetic concern by itself and sometimes due to soft tissue or limb hypertrophy..

Can any of this be dangerous or would the occasional pain I feel just be a nuisance and a cosmetic problem?

I started feeling the area swell when I were using my legs a lot, can whatever is under the birthmark be pressing on something or can the tissue itself get irritated or swollen because of overuse or from the muscles pushing it around?

Hi.

Perhaps the swelling and pain is from overuse. That is what you have also noticed. Capillary malformation is not a normal vasculature so perhaps overuse associated with increased blood flow leads to some leakage and swelling.
