Question: I fell and landed directly on my knee. No swelling. No bruising. About three days after the fall, I have a spot on my kneecap that if it is barely touched, the most severe intense pain occurs. It almost feels as if there is fluid that occasional shifts as well.

You need to have an MRI scan to find out which among these is injured. The fluid in the knee may need to be drained and infection also needs to be ruled out after a proper examination. It is best to consult an orthopedician and get the proper investigations done.
