Question: I had a TURP in January 2017. I have no bladder pain, my stream is reasonably large but I can only hold a cup of urine before having to urinate. I get up every hour to hour and a half to urinate. I am so frustrated at not being able to get enough sleep, I don't know if this is caused by a thickened bladder wall, or weak muscles, or missing muscles or ???? I had a cystoscopy in January of 2019 and the urinary tract was wide open, Even 6 months to a year after the original TURP I could only hold a cup of urine before needing to urinate.

Thanks for the query.

Based on the facts that you have posted you have undergone surgery of TURP in XXXXXXX 2017 for Benign Enlargement of Prostate (BEP) however since then you have been facing the problem of urgency and frequency for urination.

I would explain pathophysiology responsible for these symptoms:
Benign Enlargement of Prostate (BEP) causes obstruction to the flow of urine causing poor urine flow, the frequency of urination, nocturia, etc. As a natural response of the body urinary bladder contracts frequently to overcome this obstruction which leads to thickening of bladder wall and bladder develops a habit to contract even it gets filled up with the small quantity of urine which causes the symptom of frequency for urination and become irritable as well causing the symptom of urgency for urination.

Normally both these symptoms get resolved gradually after surgery of removal of the prostate (TURP) over a period of 6-10 months but in 5 -10% patient irritability of bladder fails to get resolved causing urgency for urination to continue for years.

Taking anti-muscarinic drugs like Merabegron 25mg twice daily or 50mg once daily for 3-4 months will definitely help to resolve the symptom.
