Question: I had a "mild" cardiac infaction on August 29, 2005. No stent or angioplasty was done during PCI. However, the doctor did diagnose a Left Ejection Fraction of 15% which he called Kawasaki Disease. He put me on eplerone (sp) [which he said I'd probably have to be on for one year] and Coreg. I was off the eplerone in 6 weeks, but have remained on the Coreg CR 20 mg these past 15 years. Just had a Calcium Score Test which was "0". My cardiologist keeps wanting to put me on a statin, which I resist. What is your considered opinion on my cardiac condititon?

Hello!

Welcome to Ask a Doctor service!

I passed carefully through your question and would agree with you on the fact that a statin could cause muscle paint as a common adverse effect.

From the other hand, as long as there is no atherosclerotic plaque, there is no need for stains.

In Kawasaki disease, statins are not necessary too.

For this reason, I would not recommend taking statins, unless you have an abnormal blood lipid profile ( high cholesterol or Total cholesterol/ HDL ratio).

So, you can continue with your current therapy without taking statins and periodically check your blood lipid profile every 3 months.

I would also recommend taking coenzyme Q10 and Carnitine to help improve your muscle and heart function.

Hope you will find this information helpful!

Thank you.

I do take Umbiquinol (COQ10), AMPK, Cardio Peak, Lipid Control, Red Yeast Rice. Most of the supplements are from Life Extenstion Foundation in Florida.
I am a firm believer in the benefits of Nattokinaise as well.

I am having blood work done later today. My LDL is usually higher than my cardiologist would like, but since Calcium Score is 0, I agree about no statin.
I eat mostly plant based diet and no red meat.
Thanks again for your input.

Detailed Answer:
Hello again!

Thank you for the additional information!

I think that these supplements are helpful considering your clinical situation.

Regarding your blood lipid profile, LDL is not a true indicator of dyslipidemia. Total cholesterol/HDL ratio ( normal range below 4.5) and LDL/HDL ratio ( normal range below 3.5) are the best tools to investigate for dyslipidemia.

You should discuss with your Doctor on the above issue.

I would be glad to assist you again in case of further questions or to review your performed tests results.

Let me know about everything!

Wishing all the best,

Dr.Iliri
