Question: I had a radical prostatectomy in July 2018. The pathology report came back with Stage pT3b with XXXXXXX score 8 (3+5). Bilateral seminal vesicles are positive for tumor. First PSA came back after 30 days < .1. Scheduled for second PSA in late November. Went for a second opinion and they recommended Lupron shot and adjuvant radiation now. I had the Lupron shot five weeks ago for three months. I was told impotence and libido would return 3 to 6 months later. Should I have the adjuvant radiation now and will my sex life ever return after radical prostatectomy, Lupron, and additional radiation?

Based on the facts that you have undergone Radical Prostatectomy for Cancer of prostate Stage pT3b with XXXXXXX score 8 (3+5). with involvement of seminal vesicles and facing problem of Erectile Dysfunction and Loss of libido .

Loss of sexual libido and erectile dysfunction (ED) are known side effects of radical prostatectomy and recovery from these symptoms is very slow and to be honest it is very difficult to predict about the time frame when it would happen or whether it will happen at all .

The treatment of cancer of prostate involves use of combinations of various chemotherapeutic drugs and hormones which themselves have side effects related to sexual functions .However the primary aim in management of treating cancer of the prostate is to suppress the growth of the cancer cells over looking the side effects like loss of libido ,erectile dysfunction ,loss of ejaculate etc and one has to be ready to accept the reality.

To be true it is very difficult to predict about when loss of libido ,impotence would return or whether they would return or not .

I understand. Should I get the adjuvant radiation now based on the fact I have seminal vessel penetration even though my PSA was < .1 when it was checked 30 days after my surgery? Or can you tell me if I should get the radiation at all?

Thanks for follow up .
Since seminal vesicles were involved It is always advisable to take adjuvant radiation to prevent further spread of cancer irrespective of PSA levels .
