I am 45 years old. Had symptoms at night of frequent urination, blood in urine and a fever i believe. Was put on antibiotics-cipro...urine was positive for leukocytes, nitrities and hematuria. Urine culture had no growth. CBC test and MBP test were good except for low iron. PSA was 11.4. One week later urine test was negative for hematuria, leukocytes and nitrities. Ultrasound showed enlarged prostate 6.2x3.2x4.1 with volume 43.1cc. Bladder and kidneys, etc were normal according to the doctor. DRE was done and prostate was normal according to the doctor. Will be taking another PSA when done with antibiotics. Had another urine test which was negative. Just so worried!

Brief Answer:
Elevated PSA could be mostly due to Acute Prostatitis

Detailed Answer:
Hello
Thanks for feedback .

Elevated PSA 11.4 is mostly due to Acute Prostatitis and should come done after taking antibiotics for 2-3 weeks .

Frequent urination with enlarged prostate detected on scanning needs to investigate further by doing Uroflowmetry to decide for taking alpha blockers like Tomsulosin .

Repeat PSA and consult your Urologist for further management .

Thanks and Regards.

Start taking Tomsulosin and Dutasteride once daily

Detailed Answer:
Hello
Thanks fr follow up .

You seem to have voiding symptoms due to Benign Enlarged Prostate (BEP).

Start taking combination of Tomsulosin and Dutasteride once daily (preferably at night) .

You need to monitor the size of the prostate by doing Ultrasound Scanning at every six months interval .

Repeat PSA test after six weeks as suggested earlier

As far as tomusulosin and dutasterid, my doctor only prescribe antibiotics for 4 weeks. I'm not sure why he didn't prescribe the other two. During Dre, doctor felt nothing abnormal. But ultrasound showed enlarged prostate...why is that?

Brief Answer:
My suggestions are based on Scan report and your symptoms .

Detailed Answer:
Hello
Thanks for follow up .

It is likely that he may reassess you after 6 weeks and prescribe drugs for BEP.

DRE detects presence of hard nodule (Suggestive of Cancer of Prostate ) Since he did not find any hard nodule on DRE examination he has commented as nothing abnormal .

Thanks and Regards.

A UTI can cause hives, welts, itching? Never heard of that. My urine was tested 3 weeks ago and no infection. Just trying to figure out whether these hives, itching, etc is related or not. Doctor ordered CBC and CMP blood test. Doctor said that she didn't see anything that would cause those issues from blood report.

UTI can cause the symptoms that you have mentioned .

Urine culture would be more appropriate and is confirmative than only routine urine analysis to rule out UTI.

I started on antibiotics on 7/27/20 and finished on 8/25/20. On 9/4/20 i have blood work for PSA count and doctors consult on 9/15/20. I was so anxious to know whether my PSA was going up or down, so on 8/19/20 I had blood work drawn myself for PSA count. Previously PSA count was 11.4, on 8/19/20 it was 2.4. Is that a good sign? I was still on antibiotics for about an week after that 2.4 PSA count. Do you recommend that i should take ibuprofen, naproxen, etc for my enlarge prostate, until I see the doctor on 9/15/20?

Your recent PSA is well within normal range suggesting that infection of Prostate (Acute Prostatitis ) has been completely cured

There is no need to take antibiotics now

You may continue Tomsulosin and Dutasteride once daily for enlarged prostate .

Psa is 1.1 as of 9/2/20. I go to the doctor next week to discuss. I am confused on whether I have a enlarged prostate due to a prostate infection or a enlarged prostate and a prostate infection. I thought once the infection was gone that the prostate would return to normal size. And also I had a urine test done today 9/12/20 sciatica it showed positive for nitrities ( No leukocytes). Everything else was negative. That has me a little nervous....plz advise...thanks!

Detailed Answer:
Hello

Your recent PSA is almost normal .

The infection of the prostate was the primary cause for PSA to be raised .As regards enlargement of prostate you need to do repeat Ultrasound Scanning of abdomen to assess the size of the prostate .

As regards presence of Nitrites in urine you need not worry about it as long as there are no leukocytes.

Thanks and Regards.

I have an doctor appt on 9/15/20, should I wait until then and let him prescribe me something or do I need the antibiotics now? I don't want the urine culture to be sterile( no growth) if doctor does urine culture. If this makes sense.

Tomsulosin will cause Retrograde Ejaculation .

Detailed Answer:
Hello
Thanks for follow up .

Tomsulosin has a side effect of Retrograde ejaculation .Dutasteride taken over a long period does affect sexual potency causing loss of libido .

Both the drugs are being advised and give symptomatic relief and help to improve urine flow however one has to evaluate the status of Enlarged prostate at frequent interval say every six months and may need to take a call for need for surgical intervention which depends upon the severity of symptoms , status of size of prostate and post void residual urine volume .
