The pain you are describing could be from many causes, but the first one that comes to mind is a kidney stone, which can cause sudden, severe pain on the flanks, that comes and goes in waves or spasms, and may change position as the stone travels through the ureter. You may also notice blood in the urine, but often this is not visible. Back muscle pain is possible, especially if the pain is worsened or improved with certain movements or positions, and pressing on the muscle make a difference in the pain. Other possibilities would be appendicitis, diverticulitis (an infection of an out-pouching of the intestine), and sometimes poor blood supply to the intestines or surrounding areas can cause severe pain as well.
If you are fairly certain this is not muscular, then I think it would be best to go to the ER and figure out what is causing this. They will likely want to do a CT scan, which hopefully will show the cause of this and get you started on treatment. The sooner you get started on treatment for whatever this is, the better, so I think it is best not to wait.
