Question: I had sex with a prostitute and the condom broke, had unprotected vagina sex for 2 mins. I was worried about HIV and had the girl take an oral HIV test and she tested negative. Should I start PEP?

Hello, I'm Dr. Branch, thanks for using 'Ask a Doctor'. I would not recommend PEP in this situation. First, the chances of transmitting the virus with unprotected vaginal sex during one episode is quite small (smaller than people realize, about 1 in 2500). Second, the fact that she tested negative means that as of 3 months ago, she was not infected with the virus, and almost certainly 1 month she wasn't infected as well. So the chances of her getting infected in the past month, and then infecting you, would be considered very slim, and I don't think PEP would be necessary.

If you were still concerned, PEP may still be offered to you (ideally it would be started within 72 hours of the exposure). Otherwise, you could test yourself at 1 and 3 months with a 4th generation test, which I would expect to confirm that you are negative.
