
I Have A 9 Yo Boy With An Elevated Resting Heart Rate 122 BPM At Rest

Question: I have a 9 yo boy with an elevated resting heart rate 122 BPM at rest. He has a normal 12 lead EKG, prolactin level of 35.9. his cortisol, iron, CBC, and chem 7 is normal.

Anyhow, his heart rate is not too high; its just borderline(>120 is considered tachycardia for this age)- we need not worry much as of now.

Dr Aslam, what is the significance of the elevated prolactin? does this rise because of the medication he takes? I cannot attach lab but can relay his EKG showed sinus tachycardia and his TSH and chemistry are normal. I don't know if he also has tachycardia during sleep. He does not report adverse symptoms. thanks for your help

The elevated prolactin level is probably drug related (RISPERIDAL). It is only marginally high. This needs further evaluation if the level progressively increases on sequential testing(very high levels may cause sexual dysfunction later) or if he develops additional symptoms like headache/visual disturbance. Otherwise, this should not be a cause of concern. Anyhow, you can discuss this with his physician for any possible need for drug adjustment.
