I Have A Butt Wound That Is 2 Cm X

Question: I have a butt wound that is 2 cm x 2 cm and have talked to multiple wound care specialists that have recommended Santyl that doesn't seem to be working so one recommended putting medihoney on top of the santayl but do those work together?

- Since when is the wound present?
- How did this start?
- Do you have diabetes or any other medical problems? I not checked for Diabetes please get blood sugars and HbA1c done and post reports.
- Any pus coming after pressing by sides of the wound?
- Where exactly is the wound? I mean at the pressure point, near anus? ( you may post additional photos from a distance to clarify this point.

There has to be a reason why the wound is not healing.
Hence the most probable causes I can think of as per the history you have provided are as follows:
- Pressure on the sore not allowing proper circulation and thus not allowing healing.
- Possibility of a fistula although remote must be considered in non-healing wounds near anus.
I would advise you the following in such a situation:
- Use doughnut pillows in such a way that the pressure on the wound is nil. This will help better circulation and healing.
- Use Liquid Povidone Iodine locally - no ointments to be used.
- Use small sterile gauze soaked in liquid povidone iodine as dressing. Cotton should not be used as it increases local basal metabolic rate and does not allow healing.
- Take Multivitamins and minerals with Lycopene as they help healing.
- Get Vitamin A and D done and proper replacement if found low.
If you all these things well then the possibility of early and complete healing are there.
- Always get an opinion of a General Surgeon and rule out fistula or sinus formation; if present then you will need additional treatment.
