Question: I have a cyst or lump under my skin on the right center of my back, just off the left lower corner of my right shoulder blade

The lesion you are referring to is a dermoid cyst or lipoma. Both of these are benign, not dangerous lumps. The first one is fat collected within a capsule under the skin (fat gland getting enlarged), while the last one is due to fat collection under the skin (not within a capsule).
They both can become irritated and painful but this is more likely with the dermoid cyst.

It is easy to tell one from the other by feeling the lesion. In both cases, local anesthesia and a minor surgery in the doctor's office is sufficient for removal in case it is bothering you (it takes less than an hour to remove it).
