I Have A Dental Cyst With A Biopsy That Says
Question: I have a dental cyst with a biopsy that says 'Inflamed cyst with focal ameloblastic islands. There are marked inflammatory reactive changes present. Such changes can alter the histology of the lining... The specimen consists of cystic epithelium which is hyperplastic, proliferative and marked inflamed. There are scattered ameloblastic islands in the wall.
Recommended additional biopsies of the lesion before definitive treatment.
This will give more of a definitive diagnosis of ameloblastoma.
Okay...I haven't gotten the 2nd biopsy yet. My cyst is 4.5cm. It has resorbed roots of two teeth. Half of another and it touches the next tooth in the back. It extends forward to the midline of my mouth with bone loss but with full roots of my front teeth. The cyst is maxillary. If the biopsy showsit to be unicystic, then that is better than the other sub-types.
My question is, do wide resections require bone grafting?
Do all the teeth need to be extracted?
What are your recommendations for my case, what should I do next?
Further information...the cyst has not moved any of my teeth and all are solid with no movement. No pain. No facial swelling.

Based on your history and description, i cannot give you the diagnosis and treatment plan. If it is a cyst, the treatment modality will be of Enucleation or complete removal of the cyst. If it is a ameloblastoma, then the treatment option will differ. It has to be managed with surgical excision or Resection of the area of involvement. To decide this you need to get the X ray and biopsy done to confirm the diagnosis. Consult an Oral and maxillofacial surgeon and get it evaluated. respected sir, you are telling it's cyst and your histopathology report is saying it as ameloblastoma. Ameloblastoma is a benign tumor of tooth origin. Regarding your teeth, if the teeth roots are resorbed, then those teeth needs to be removed. If it is ameloblastoma then all the involved teeth needs to be removed irrespective of whether the teeth have pain or not, whether it's moved or not. To tell whether you require grafting or not we need to see the radiograph or x ray images.
Following surgical resection of the ameloblastoma, even part of the normal bone structure also needs to be removed along with the ameloblastoma it results in facial disfugurment that requires grafting with scapula, iliac crest or fibula.
