Question: I have a feeling of constipation but different. I can't have a bowel movement-but cannot pass gas. I have had soreness in an area around the anus in kind of a horseshoe figure going above the top. Also having pain when I try to have bowel movement- only a little bit comes out-like diarrhea. Feels like I'm ready to have a bowel movement-but can't which creates pain. Also feel like I have to urinate but can't- sometimes can urinate.

I've gone through your query and understand your concerns...

Well, the pathology/contributory factor could be impacted motion itself as it can cause excessive straining which in turn results in the injury of the colon and rectum, and this is felt as such kind of pain...

The fact that a little bit of motion comes out like diarrhoea is even more suggestive of this (impacted fecal matter)...Now the cause of impacted faecal matter could anywhere from longterm constipation, hard faecal matter, inflammatory bowel disease, gut motility disorders, sedentary lifestyle, changes in dietary habits and several other conditions...

The horseshoe kind of figure that you described could be due to haemorrhoids/piles (which can develop secondary to such condition)...

Since you've mentioned already taking the stool softener and fluids, I'd recommend the following:

-Take Sitz bath twice to thrice daily (sit in a tub of lukewarm water putting some salt in it)...You can watch a tutorial video of Sitz bath online for proper techniques...

-You should go for a per-rectal examination (by any medical doctor, preferably a Gastroenterologist/Gastro Surgeon) followed by P.C.Enema treatment initially, ideally it should be given by the medical personnel, so it'd be better to get done in any nearby clinic/urgent care...

-Apart from this, please try not to strain while passing motion...

-In case you've already undergone any routine colonoscopy check-up earlier (and if they're clear), then I wouldn't think of getting another one immediately, however, if you haven't, then this is the right time to get a check colonoscopy done...

-Drink plenty of water(at least 8-10cups per day), have plenty of fresh green vegetables, simple homemade foods, yoghurt, salads, oats, muesli, high fibre diet, avoid any form of spicy food...avoid smoking/alcohol strictly if at all do...

Now, the urinary concern may or may not be related to this, at first let the faecal impaction get cleared and the stress relieved, then observe the urinary stream, if still recurring then a Urological check-up along with a few investigations (particularly serum PSA, USG Abdomen with special focus on Kidney, Ureter, Bladder and Prostate) should be done to look into it in details...
