MD internal medicine...
Detailed Answer:
Hi,
I understand and empathize with your problem. You have been suffering from low grade fever for a month. The causes are multiple ranging from infections, inflammation, immunocompromised status, tumors and so on. A physical examination and battery of tests - blood tests and radiological tests are necessary to isolate the problem. If the current doctor's are unable to find a cause, it might require a multispeciality approach - where group of doctors from different specialization work together and isolate the cause.
The first step will be to visit a physician, a board certified MD internal medicine specialist. If deemed necessary, other specialist such as infectious disease specialist may be needed.
Hope this information helps. Let me know if you need clarifications.

I did see MD internal medicines and he did all blood test and ruled out any immunology diseases including RA.
He didn't carry out any radiology tests and said I should go to gynaec and got a scan done... but my feeling is it is something else ... I will go to gynaec in coming week but if negative what next ... I think there is something being missed out completely.
The routine approach isn't working to identify the problem.
Can you explain all your symptoms?
Detailed Answer:
Hi,
Persistent fever has multiple causes. History of illness, examination and investigation are means to obtain clues / diagnosis of the problem. The routine tests and immunological tests have provided no clues yet. And as I mentioned earlier, a step wise approach will be the best means to the diagnosis.
I am not sure why your doctor's order for a gynec opinion. Perhaps they wish to know if the fever is from genitourinary system. An ultrasound scan is definitely indicated.
Can you explain the symptoms?
1. Since when the problem started - did it start abruptly or you had gradual development of symptoms?
2. Do you feel feverish throughout the day or is the fever intermittent in nature? If it's intermittent - is the fever more in the morning / evening?
3. Do you have chills and shivering with fever?
4. Do you have joint and body pains?
5. What other associated complaints do you have - abdominal pain, cold, cough, vomiting, loose stools, painful urination and so on?
Regards

Need further testing / evaluation....
Detailed Answer:
Hi,
Thanks for providing details. From the given details, you have fever that rises in the evening with body pains and no other symptoms. The current investigations did not reveal any significant abnormalities. In this circumstance, there is need for more extensive evaluation. As mentioned earlier I will recommend multispeciality approach. Besides gynecology opinion, I will also suggest taking an opinion from infectious disease specialist - viral serology and other infection serology marker are recommended.
Hope this helped. Let me know if you need any more information.
