Intermittent spasms and pain in the mid back region can either be due to kidney related issues like stones or spine-muscle related issues like osteoporosis, disc prolapse etc.
I see that a sonogram has been done before in the emergency room. However, the sonogram may not always detect kidney/ureteric stones especially if the size of the stone is small.
A CT scan of kidney, ureter and bladder (KUB) will provide a more detailed and enhanced view. CT scan will also help to visualize any other underlying kidney pathology. CT spine can also be done along with the CT KUB to rule out spinal issues. Depending on your general health status, the doctor can decide on doing CT scan with or without contrast enhancement.
It is also recommended to get a urine analysis and culture done to rule out any underlying kidney/bladder infection.
You can follow up with a urologist for detailed evaluation. You may also follow up with your primary provider who can do the initial evaluation and do a referral to the specialist if needed.
Meanwhile, you can take Tylenol (available over the counter) for pain relief.
