Question: I have a pain that starts in my right hip and goes down into my knee, shin and into my ankle joint. I went through 6 weeks of physical therapy and that helped, but the pain still persists. I have an appointment with an orthopedic specialist next week. I nervous about this appointment. What should I expect?

Well, to some extent, any pinched nerves at the lumbosacral spine level (the back) to the downwards can give rise to such pain radiating downwards and the discomfort...the fact that the physical therapy helped is even more suggestive of this...

First of all, please avoid any strenuous activities, weight bearing, for now, avoid back/forward bending also...When sitting, sit with an erect spine (a good back-support i.e. lumbar belt can be used)...

If you're in a desk job, taking short duration frequent breaks would be helpful...just walk around for 5minutes after every hour of sitting down...

It's good that the Orthopedician appointment is already set...nothing to be nervous about it... The Orthopedician will evaluate you further...a few relevant investigations i.e. an X-ray of the L-S spine along with CBC, serum Calcium, Vit D3, Vit-B12 levels, Uric acid, ESR to look into it further...based on these findings, an MRI-of the L-S spine may also need to be done to look into any possible nerve root entrapments which could be responsible for all these symptoms...

The physical therapy may need to be continued even for a longer duration...along with it, the lumbar belt should also need to be put on a daily basis...Meanwhile applying a mild pain-killer spray (eg. Diclofenac spray) around the affected area of the back would be helpful (avoid any kind of massaging without supervision)...
