I Have A Problem With Dizziness Since My Bypass Surgery

Question: I have a problem with dizziness since my bypass surgery in July 2017. I had an elevated BP for awhile an was put on BP meds. Then my BP dropped too low and I was taken off. that seems to be when the dizziness started. I had a sinus infection fall of 2017 and fall of 2018. I am on Flonase now. My ears are very transient as my BP seems to have become. They feel like there fluid in them and other times not. I do have issues with my ears and my allergies but my ears can be clear now and there no fluid. I had sinus study and everything OK. My sinuses just drain in my ears. It an old problem but does not seem to be related to my present problem. I used to be only dizzy in the am but now it is all day. My neurologist say my circulation is not what it should be. Wants me to wear compression hose up to my groin. How dizzy I seem to be related to my positioning. Yesterday was the worst so far. a little history first. I was in a car accicent in the 80's and lost the use of my arms for a month. Since the bypass surgery I have been losing strength in my upper arms. I was going through my recipes reaching up into a bookshelf, my arms above my head. all at once I realized I felt like I was about to faint. My arms felt like lead and like all my blood had left both my arms and head. My husband helped me to a chair. My BP pushing above 140/70 again, My BO was 158/78 5:30 this am when I got up. P was 56, neither are normal for me. P low BP high? to top it off, I feel confused at times. I have been in ER and in the hospital but nothing shows up then. I don't know what going on. I due know stress makes it worst but that to be expected. I am having to use two canes to walk, help.

After reading your detailed history, I am of the impression that you probably have orthostatic intolerance which means that your BP goes down while going up standing from the lying down position. Your treating doctor rightly said that high compression graduated stockings will help. Also, you should maintain adequate hydration. Further, you should stand up gradually and not abruptly from the supine position which may also help in alleviating your symptoms.

No, my BP goes up when I get up, plus My left and right arm have quite different BP. I can have this problem all day especially if I am stressed. My left arm was 128/106 my right 111/55 Sat. am. Sunday I was better. the left was 132/61 right was 115/57. I can feel a little dizzy when I get up but get very dizzy when I try to hurray around in am. My later BP was better but still dizzy but not so bad as the day go on. I use to be only dizzy in the am but it can stretch all day now.

End of May 2017 I was working at a nursing home in Las Cruces. (I have not been able to work the Roswell nursing homes because I was blacklisted for testifying against them in 1997.) We had to rent a place to live in Las Cruces to work the job. I was moved to a different hall and different pt. while still on orientation, I was under a lot of stress. My boss knew it been seven years since I worked as a nurse and had the promise to be kind but it wasn’t happening. One night I went to the BR I had an H/A and felt stressed out. I happened to look in the mirror and the capillaries were bursting in my right eye. A few nights later I now had a new admit and other issues going on and I really felt overwhelmed. I had an H/A again. I went to the BR. and the capillaries in my left eye where breaking again. I had a tech take my BP and it was in the 170’s. I didn’t feel well and decided I had enough of this job. I didn’t need a stroke. The nurse orientating me took over and I left.

Tonight, July 23, 2017, My throat felt like I had a sore throat but especially on the left side. Tighter than anything. My chest hurt in a way I never hurt before. NO SOB though. I do have GI issues but this was different. I took some cayenne pepper and the feeling went away. If it been indigestion, it wouldn’t have. In fact, the cayenne would have made my indigestion worse.

Thanks for being in follow-up.

Firstly, sorry for a delayed reply as I was busy due to some urgent work. I have gone through your detailed history. The symptoms appear more to be due to mental stress and anxiety, rather than a TIA or stroke. Further, if a clinical TIA is actually a stroke, then only it is seen in MRI; per se, TIA is not seen in MRI if it did not cause any brain ischemia. Though, I think these symptoms are non-specific to be ascribed to TIA. Concerning dizziness, fall in BP after 3 minutes of standing is taken for orthostatic intolerance and not just after immediately standing. Your dizziness symptoms appear to be due to orthostatic intolerance and probably due to that only you were prescribed compression stockings.You should try wearing them, maintaining adequate hydration and doing regular meditation/yoga and hopefully you should do well after that. These appear to be minor things to do, but may do wonders for you.

i never thought it was a stroke but a TIA maybe. I will try the compression hose but I didn't appreciate the comments about it being all in my head, nerves? The local ENT thinks I have some real nerve damage in my head, not psych. issues.
