Noted the short history and seen the photo.
My thoughts:
If this is long standing of few weeks or months then the most probable cause of such sprouting granulation is either a sinus or a fistula in ano.
Other causes can be granuloma after a boil; rubbed papilloma and so on.
This can be confirmed by the following ways:
Local examination and probing by a Doctor, preferably a General Surgeon.
If sinus or fistula is suspected then per-rectal examination, MRI Fistulogram to confirm the diagnosis and to know whether high or low.
Please get these done and give me feedback.
Give detailed history.
Post clear and focused photo.
Apply Sterile gauze dressing soaked in liquid Povidone Iodine.

I have attached a better picture.
It started a year back before there used to be blood coming out, now it does but very little.
It itches but very rarely, but rarely also burns, at times the blood comes out.
