Question: I have a rash that started in my back and has now spread to my chest. it has changed the color of my skin and it itches really bad.

I see the photo you attached and it looks like tinea versicolor. It gives itching of the skin and discoloration of the skin.
Tinea versicolor is a common fungal infection of the skin.
This fungus affects the pigmentation if the skin and that's why you are having the discolored patches.
You can use antifungal topical ointment such ketoconazole gel or shampoo or other antifungal ointments such fluconazole or intraconazole all over the affected skin.
Apply the cream on skin until it does not itches anymore.
