Question: I have a red bump or rash on my right knee with thin white discolored skin surrounding it. My dermatologist has tried liquid nitrogen, clobetasol 0.05% and a steroid injection. It just won't go away. What could it be?

Hello and welcome to ask a doctor service. I have gone through your query and viewed the Image.

It seems like there is atrophy at the site you mentioned or it is a thin scar. It looks red because of thin skin, perhaps. Thinning/ scaring could be a side effect using potent topical steroids and using liquid nitrogen. I suggest you to use a moisturizer like liquid paraffin or some other bland moisturizer and hopefully it will improve over time as the skin restores itself.

what is actinic keratosis? I read it is caused by exposure to sunlight. I do not spend that much time out in the sun, especially not exposing my knees. As I have said before, it looked like that before any steroids were prescribed. Could a disease or medical condition cause skin atrophy?

yes, actinic keratosis is sun induced and mostly presents as scaly, erythematous lesions on exposed sites like arms, legs, face. They happen from chronic, cummulative sun exposure (after years of sun exposure) and on a skin that usually shows other signs of sun damage. I just kept it as a possibility with atrophy, as a side effect of treatment rather than being primary. Other possibilities that I can think of are perhaps, erythema elevatum diutinum (EED) and discoid lupus erythematosus (DLE) but mind you these are just possibilities based on morphology and require histopathology confirmation. Both of these conditions have atrophy as part of the pathology.
