Question: I have a small cluster of bumps around the base of my penis head. I am also uncircumcised.

Hello! These lesions came about sometime early last year. However, they disappear then reappear after some time.
In the images I attached, one photo shows the small red lesions and the other shows a red patch at the base of the head. That red patch also disappears and reappears from time to time

Good day and thank you for sending me the pictures.
These looks like minor irritation and should go away in a one to two days. Make sure you have good hygiene and washing your organ with mild soap and water and not let perspiration or sweat dry out on it own.
it doesn't look like it's a sexually transmitted disease however if it is persistent, causes pain or scars when it is healing can i suggest you see your local urologist so we may do a physical examination and possibly a blood exam to rule out herpes virus infection.
