I have gone through your he pictures.
The lesion is typical of ringworm or tinea.
You can try over the counter available antifugal-steroid combination like clotrimazole/betamethasone. You have to keep the area dry.
If there is itching antihistamine group of drugs like cetrezine may be taken.

I have tried lotrimin ultra and miconazole, as directed, with minimal improvement. Is clomitrazole the next best choice if those have failed? It does not appear that I can get the combo med over the counter
