Hello, Thank you for using the "Ask a doctor" service.
I reviewed your photo.
Why do you say it is a hematoma?
Can you explain to me what happened with your hand?
Did you fell or hit it somewhere? When did it happen? Can you move your fingers?
Please provide me some information that you think can be relevant so that I can advise properly.

I believe I have a sub-dermal hematoma on the back of my hand . Not sure what else would raise my skin like this. It is about 2" X 1.5" and 1/2" tall. I did not hit my hand that I know of. It didn't hurt. It's been about 5 hours and it hasn't gotten smaller. What do you do in a case like this? I'm 66, on a blood thinner, I have high BP. I've always bruised easily but not like this.
Brief Answer:
Advises what to do but also check with your doctor the doses of medicines

Hello,
Thank you for the follow up question.
Since you are on a blood thinner the situation becomes tricky.
Normally it is goed to take rest and not use/move the hand that much so the vessels that are damaged will heal and the subdermal bleeding will stop. Also apply ice or cold pack for 20 minutes at a time, 4 to 8 times a day, compress by using elastic bandages, and keep the hand as much as possible above the level of the heart.
By doing this it should not become bigger.
Also a few hours are not enough to become smaller but in 3-4 days should start changing color and become smaller.
If that does not happen, then you should see a local doctor because if there is too much blood then it is not possible for the whole blood to be absorbed by the body itself, maybe has created a pocket and need to be drained.
Also since you are on blood thinners you should check the doses by your doctor. If you are having such a spontaneous hematoma then it most probably means that you are taking higher doses than you need. So take care of yourself about this because you never know next time which blood vessel is going to bleed next time.
I hope I have answered your question. Let me know if I can assist you further.
Kind regards,
Dr. Papaqako
