Question: I have achy swollen joints in hands and toes Hands are very distorted and enlarged I have pain all the time I am very sensitive to cold and am fatigued much of the time. RA, b12 deficiency and thyroid have been eliminated . What do I ask about next?

HI and welcome.
YOur symptoms are suggestive of generalized arthritis which is most commonly seen in autoimmune disorders: rehumatoid arhritis, osteoarhritis or poliarthritis nodosum.You may also have fibromyalgia or systemic lupus.

I dont think this is related to thyroid or vitamin defficiency so you should focus on autoimmune disorders. Beside RA factor, you should check ANA (antinuclear antibody), cyclic citrullinated peptide (CCP) antibody, HLA-B27, uric levels and viral markers.
ALso, xrays of hands would be useful.

If all these are fine, then you should start with exercises, hav eantiinflammatory medications such as metotrexat or decortin,but it should be discussed with immunologist.
