First of all, if it has not been done already, it is important to rule out heart disease, e.g. by ECG. If the pain is exacerbated by activity it suggests heart disease rather than an acid problem. If your symptoms are aggrevated by spicy and smoked foods, being in a reclined position, and associated with reflux, it suggests an acid problem. Then you can try avoiding late meals, smoked and spicy food, smoking and drinking, and elevating your head rest. If that doesn't work, try a proton pump inhibitor for 4weeks.
