Question: I have already got really helpful answers from 2 doctors but want to get a second opinion from a OBGN I had an episode about 4years ago of severe burning in the vulva. After many trips to different GPs they decided that it was nerve pain and over sensitive nerves. I was prescribed 20mg amytripteline and anesthetic gel. After couple of months it went totally but I was advised o keep taking the amytripteline and put the dose down to 10mg. I looked on the internet for what this may be and saw a whole string of depressing sites about vulvodynia and chronic pain. I was totally without any pain for 4 yrs until now. it is slightly better today and I am wondering if this could be the above and if so I am in some sort of flare. do you know anything about this ? any advise would be realyy helpful. When I wrote this the burning was quite bad , but seems to have gone now and just left a slight itch and pins and needles. Could this go away again for a long period and just flare up sometimes?

I have read your query and here is my advice.

* As per my clinical experience, the vulvodynia is a non specific condition, where due to
unknown reasons you get nerve irritation, varying degree of burning and other stuffs.

* Usually no evaluations pick up this issue and there are no definite therapies for that.
It can show up at varying periods of times, more commonly during stress.

* My guidelines for particular issue

- Maintain hydration with plenty of liquids.
- Keep genital hygiene at its best.
- Avoid stress and anxiety.
- Manage to have sound sleep of 7-8 hours.
- Keep your blood sugar under control.

* Consider problem and consult your doctor only when there is itching, some skin
changes or more burning ( as the more you will focus and think about it, more it will
distress you without reasons ).

Thank you Dr this is exactly what my husband said, the more you focus on it the worse it will be. I do not feel any stress of anxiety beforehand but got very depressed when it returned. I eat healthy maybe not drink enough water....so will try. I do yoga twice a week. \In your opinion would you consider what I have been going through as a flare up of symptoms as I had not had any of these symptoms since 2015, they just appeared from no where. Have you come across this much and can it just go away at anytime? I have heard people suffer for years with no relief, but I find that difficult to believe that symptoms cannot be treated and managed.

previous one, means that your body is fighting much better as compared to previous
episode.

* There are many instances in the medical science where there is no cause or remedy.
It is possible for this issue to disappear anytime without you notice.

* The medicine given to you is also non specific.
I would suggest to use aloe vera gel over the part and get help from herbal approach
to some extent.

Have you come across Vulvodynia or fibromyalgia or any of these neurological symptoms in ur practice ? As I am feeling that you maybe think it is all in the mind , my Hp put me on amitripilyne to stop the sensation of pain and severe itch are you saying I should not be taking this.

I have came across around 10 cases in my career and out of which almost all are cured with symptomatic treatment for a varying period of 4-6 weeks and relapse of symptoms in one case which was also cured.

I am not conveying you that this is psychological.
Condition is from unknown causes and undefined management.

And that is my main focus to keep least stress for outcome, it will surely pass out but the duration is unknown.

In my practise, along with anti histamine for itching, supportive medications for pain relief if much pain, eradication of any possible skin infection ; main focus is to concentrate on deep breathing exercises, YOGA on daily basis which supports great for early recovery.

Wish this information will make the issue much clear and still in doubts please feel free to discuss ahead.
