The heterogeneous thyroid gland in ultrasound is suggestive for chronic autoimmune thyroiditis that means the body produces some antibodies that destroy slowly the thyroid gland.
When more than 80% of the thyroid gland is destroyed, hypothyroidism or underactive thyroid is installed and at this moment thyroid hormones should be taken by mouth.
There is no treatment to lower the antibodies and to prevent thyroid destruction.
For the moment the thyroid function should have been within the normal ranges that is why your physician didn't start treatment.
You should check TSH every 6 months and start levothyroxine once it is elevated.
