Question: I have been on Doslupin for 6/7 yeats 100mg pm,I kept asking for last 2 years can I go back on Citalopramas I can't explain I felt better on Citalopram ??my GO said it will be to difficult for me to change (I am a very anxious lady I have been on different AD for 35years bad childhood marriage not helping ETC,I spoke to new GP as my own in available same practice He told me to in crease by 25mg which I did for 10 days,Rang my GPS to get follow-up to discuss but had to settle for another GO on later date all appointments are now Telphone only,This time it is Lady she said go down to 4 Dolsu for 1week gave me Risperidone 0.5mg (I've not ever had this )Instructed to take 4 Dolsu and 1 Ris 0.5mg p.m.for 2 wks then 3 Doslu + 1 Ris for 2wks,Then 3 Dolsu and 2 Ris 0.5mg 1 am and 1 pm, I am now on week 3 about to start 1Tis am and 1 pm,I rang my GP last week as bad bad diarrhoea, only got to speak to receptionist who got to speak to lady Doc who said it's ok just IBS,This is why I am asking for advice please regards

I read your question carefully. From what I understand you were on Citalopram in the past then was switched to Dosulepin. It is not clear why that was done since you say to have felt better with Citalopram. Citalopram is a much safer drug than Dosulepin so if it was effective I do not understand the switch. Dosulepin is rarely used nowadays because of more frequent side effects and no significant added benefit compared to other antidepressants. Citalopram on the other hand is one of the antidepressants with fewer side effects, widely used for treatment of anxiety.
So my advice would be to actually go back to Citalopram, but of course it must be discussed with the GP why the switch was effected, perhaps there is something i am not aware off, so you must ask for some explanation as at the end of the day your doctor is the one who has te release the prescription.
As for Risperidone, while I would try it only if other alternatives were not effective, it is unlikely for it to be the cause of your diarrhea, it is not a frequent side effect and you are on a small dosage.

Naturally I can't say exactly what the reasoning of your doctor for starting Risperidone was, I can only make assumptions. It is used at times at low dosages for treatment of anxiety. It is not an indication for which it is officially approved and it is not a first choice drug for anxiety, but it is a common practice.
So I can assume that it was used either because it was deemed that Dosulepin wasn't effective enough and an additional drug was needed, or because as you suggest the GP intends to switch to another antidepressant like Citalopram and is using Risperidone to control anxiety during this bridging period while Dosulepin is reduced gradually and another drug is started.

I wish to thank you for your time and patience .I dont know if I have another question in my package .But I just wanted to say a big thank you I prob like many others havent been able to get a face to face with a GP for weeks .I have got a telephone appointment with the Lady doctor next monday this is the GP who has put me on Ris I am feel much happier than I have in poss 6 years. As I said Risperidone is not a first choice drug for anxiety, usually is used in addition to antidepressants when they do not work. I think you should continue it for the moment until you stop Dosulepin and are started on another antidepressant (Citalopram or a similar drug of the same class). Once you are on the other depressant, the decision should be made based on your condition. If you are feeling well then it should be attempted to stop Risperidone gradually and evaluate how you're feeling. If anxiety is well controlled with Citalopram alone there is no reason to continue Risperidone in the long term.
