Please note, blood from urine and blood from stool may or may not be interrelated...As I see in your medication list there are no blood thinners mentioned, so it could be due to some other reasons...

First of all, when you said blood in stool, did you mean through the colostomy or down below? Either way, although it was clear 2 years back, a prompt check-up at the ER with some further investigations (namely, Hemogram, Liver and kidney function test, coagulation factor study) are required at this point.

The urine needs to be analyzed as well...Urine routine microscopy, culture, and cytology study should be done. Along with this, a CAT scan of the whole abdomen with KUB (kidney ureter and bladder) should be done for a thorough analysis.

Meanwhile please stay well hydrated by drinking water and electrolyte-rich fluids, and call up urgent care for further assistance...
