Question: I have daily migraines. I'm on topiramate 50mg twice daily. Could I up my dose to 150mg by taking 2 50mg in the AM and 1 50mg in the PM ( or vice versa?) I can't see my neuro for a few months

The dosage can only be increased weekly in increments of 25 mg/day in order to avoid adverse effects (like severe skin/allergic reactions) from the medication. That is, in the first week you can only increase 25 mg/day (which would amount to a total daily dose of 125 mg), which can be divided into AM or PM according to your convenience. On the 2 nd week, the dose can further be increased to 150 mg/gay.
The decision to increase dosage depends greatly upon your clinical outcome. It should only be done under the guidance of the treating physicians. You can try contacting your neurologist by phone to see if a dosage adjustment is best for you.
