
I Have Ear Fluid In Both My Ears. I Feel

Question: I have ear fluid in both my ears. I feel a bubble sensation in my right ear very frequently. I saw a doctor here and he gave me montelukast 10 mg and fluticasone propionate nasal spray to use for 3 months. I also had allergies for some months, I would wake up with congested nose and sneezing and it would get better during the day.
I wanted to get a second opinion because I don’t want to use montelukast because of certain side effects I read about with prolonged use.

According to your history, you seem to be suffering from three issues. Allergic rhinitis, Eustachian tube (the tube connecting nose and the ear) block and the serous otitis media (accumulation of fluid in the middle ear).
The problem in the ears are secondary to the nasal allergy and nasal congestion. Unless the nasal congestion doesn’t get relieved, the Eustachian tube would not open up and the fluid built up in the middle ear will not clear off.
My suggestions to you are:
1. Use Xylometazoline nasal spray or drops, followed by steam inhalation after 10 minutes. This would help in relieving the nasal congestion and opening up of the Eustachian tube. The above medicine should not be used for more than 5 days. And, it’s available over the counter.
2. Continue using Fluticasone nasal spray 2 times a day, without fail.
3. A mucolytic such as bromhexine can be used to lighten the secretions in the middle ear and easy clearance of the same.
4. Do steam inhalation 3-4 times a day for 3-5 minutes. Drink frequent sips of warm water.
5. Avoid smoking, coffee, chilled food and beverages.
6. Avoid sleeping in front of the AC blower or directly under the fan.
7. Please stop Monteleukast, as it is not of much value in this condition.
