I Have Extreme Flatulance. Extreme To The Point Of Not
Measures to help with flatulence and further tests explained in detail below
Detailed Answer:
Hello and welcome to "Ask a Doctor" service,
I carefully read your query and understand your concern with increased gas.
To treat flatulence it is important to try to identify the cause of it and if no cause is identified or the cause is untreatable then treatment should focus on controlling the amount of gas in the gastrointestinal system.
As for the cause of flatulence- GERD (acid reflux from the stomach) may be a cause so having regular antiacids (especially those with simethicone such as gas-x) is recommended.
Also if the pancreas is not producing enough enzymes malabsorption may happen and flatulence as well so it is important to consider it in the differential diagnosis. In this case, pancreatic enzymes are taken orally to help with the symptoms.
Omepazole may often cause flatulence. Its use is important in Barret Esophagus so combining it with regular use of simethicone (gas-x) may reduce flatulence.
Milk intolerance and other food intolerances should be considered and you should avoid food that makes your symptoms worse.
You have lost 25 pounds recently, this definitely needs investigation so a repeat colonoscopy is recommended, CBC, ERS and possibly a TSH and FT4 blood tests to rule out thyroid problems. I would also recommend some stool tests if not done before such as stool for ova and parasite, fat amount in stool, stool microscopy and stool culture.
To conclude: treating flatulence may be challenging but some recommendations are:
- avoiding beans and food with increased fiber or fiber supplements
- you can take beano tablets which contain alpha-galactosidase and may help with gas from vegetable, fruit, and fiber present in the diet
- it is recommended to avoid milk but you can take cheese and yogurt
- gas-x contains simethicone and may help with gas, you may choose to use it regularly
- probiotics may also help with the normal flora
- eat slowly, chew food well and do not drink from a straw and do not chew gums- this will decrease the amount of air going in your stomach with food
- avoid sweetened food (sugary drinks), avoid fatty food as well
- a healthy lifestyle helps the function of the bowel (being active and exercising regularly, avoid smoking and alcohol)
- some tests may be indicated as mentioned above to identify treatable causes of flatulence
I hope this answers your query and is helpful to you. I remain at your disposal in case further medical assistance is needed.
Kind regards,
