I Have Extreme Flatulance. Extreme To The Point Of Not

Question: I have extreme flatulance. Extreme to the point of not being able to attend social events. All day long and at night. I have Barrets Esophagus and have been taking Dmeprazole in the morning for about 1 1/2 years. I roughly follow the FOD diet to avoid gassy foods. I have recently lost 25 pounds. 1/3 has no smell and 2/3s are foul smelling like methane. No pain. BMs normal. Color yellow gold. Soft. Take Gas-x when I have a business meeting but that only decreases burping. Have had endoscopy recently and colonoscopy year ago. Had 12 polyps. Genetic test was negative for polyposis. Next one in September. Any differential help??

As for the cause of flatulence- GERD (acid reflux from the stomach) may be a cause so having regular antiacids (especially those with simethicone such as gas-x) is recommended.
Also if the pancreas is not producing enough enzymes malabsorption may happen and flatulence as well so it is important to consider it in the differential diagnosis. In this case, pancreatic enzymes are taken orally to help with the symptoms.
Omepazole may often cause flatulence. Its use is important in Barret Esophagus so combining it with regular use of simethicone (gas-x) may reduce flatulence.
Milk intolerance and other food intolerances should be considered and you should avoid food that makes your symptoms worse.
You have lost 25 pounds recently, this definitely needs investigation so a repeat colonoscopy is recommended, CBC, ERS and possibly a TSH and FT4 blood tests to rule out thyroid problems. I would also recommend some stool tests if not done before such as stool for ova and parasite, fat amount in stool, stool microscopy and stool culture.
