Question: Hi Sir, I have facing issue with hemorrhoids from the last one month. At early days... I am suffering with pain and observed some external hemorrhoids. I have started using Himalaya pilex tablets. From the last 10 days... I observed no hemorrhoids externally and no pain. But from yesterday... observed full bleeding while going stool without any pain. May I know what's the issue. Thank you

Brief Answer:
Fissure with piles is a possibility as per your history.

Detailed Answer:
Hi.
Thanks for your query.
Noted the history well.
Painful defecation with small sentinel piles is indicative of fissure-in-ano.
There can be additional piles also called as Hemorrhoids. Piles case painless bleeding. Pain is present only if the pile is prolapsed and keeps outside of anus.
Hence I would advise you to get examined by per-rectal digital palpation and ano-proctoscopy to know the exact findings so that a proper further management can be done.
Medical management starts with keeping to stools soft.
The best way is to take Isabgol husk, best one is Fabolite powder. Start taking half tsf in 200 ml water with food. This will keep the stool hydrated, soft hence easily to pass. No straining at all. But remember you may have to increase/adjust the doses to suit your needs.
Avoid the foods and beverages that cause constipation.
Further management depends upon the findings as mentioned above.

I hope this answers your query.
