I Have Gassy Feeling In My Chest And Lot Of

default
Posted on Mon, 8 Jul 2019
Question: I have gassy feeling in my chest and lot of burping. I am not sure something triggers it and it comes out of random, even after waking up in the morning. Its been going for 3 months now and its not getting any better. I have been suffering from lot of burps since Feb and this has caused my throat to be sore. I also have mild abdominal discomfort (its more like poking sensation) mostly on the upper right side and below my chest. I also have a back discomfort and again not sure if its due to the abdominal pain (this is not bothersome, but its there).

I am 28 year old male. Weight has been consistent for the last 3 months. No issues with diet and motion (mild constipation). All the issues started after consuming eggs which were 4 weeks old ( i did not know this while eating). I also had UTI last year and then sinus infection for which I had taken 2 courses of antibiotics under 3 months, maybe this could be reason for my issues.

I have also developed sensitivity to certain foods now, again not sure if its GERD or what. I am trying to find what is causing the issues.

I have tried changing my diet but the symptoms are the same, burping and gassy feeling/burning in the chest. I also have have mild pain in the right side of the abdomen which is vague and not in one single position. It comes for a min or so and goes away and on a scale of 10 I would 1-2 XXXXXXX It kind of feels like gas passing through the bile ducts but I know it should not happen.

I have had Upper GI Endoscopy with biopsy, CT scan with contrast, US of abdomen, MRI of Spine, numerous blood works: CBC,LFT,lipase, amylase, urine culture, stools tests for H-pylori, candida all of them have come back as normal.

I have been on Omeprazole and Sucralfate for some time and its not helping.

I have attached an image to show where the discomfort and gassy feeling is. Its in the attach report section. Also, when i press down hard on the middle of my abdomen I can feel some discomfort on my back.

I am afraid this could be due to some obstruction of bile ducts (pancreas, gallbladder) or SIBO.

I dont smoke and drink occasionally.

Also, should I get additional testing done? Like MRCP or HIDA scan or breath test for H-pylori or SIBO?

What should I ask my doctor like why my symptoms are not going away after trying out different avenues.

Brief Answer:
It could be due to Acid peptic disease

Detailed Answer:
Hi
I saw the picture.
Your Complaint is mainly on the right side of the Abdomen.
As per your description,it looks like Acid peptic disease.
As your Upper GI endoscopy and CT scan are normal, there is.no need.for further investigations.
Stop Alcohol completely.
Avoid caffeine products and Carbonated drinks.
Stop spicy foods.
Take small meals frequently.
Be stress free.
I prescribe Rabeprazole along with Levosulpiride to my patients with similar complaints.Sucralfate along with Oxetacaine is more beneficial.
Any obstruction in the Bile duct will lead to Jaundice. As it is not there, obstruction is excluded.
You should avoid pain killers.
The symptoms that you have mentioned are Non specific. As all investigations are normal,it can be Acid peptic disease.
Take Yogurt/curd regularly
Nothing to worry. It will come down.
Hope I answered your query. If you have any questions,I will clarify further.
Wish you good health

So, is excessive belching and flatulence common with peptic acid? What could have caused the ulcer?
I now have food sensitivities , I cant eat bell pepper or eggplant, it really irritates my stomach and I either get diarrhea or vomiting. I used to really love eating the 2 vegetables, now all of a sudden i cannot eat them, even garlic.

Dr, I saw my gastro today, he has asked me to get a BRAVO endoscopy done to rule out if the issues are caused by acid.
Even, he has ruled out bile duct issues as lot of tests including imaging, stool and blood works have all come back as normal .

He thinks this is either due to acidity or excess gas but he wants to rule out the acid part.

Brief Answer:
It could be due to reflux

Hi
Discomfort in the chest could be due to reflux of gastric juice into the esophagus. It is part of Gastroesophageal. reflux disease (GERD)..
Rabeprazole along with Domperidone will give excellent results.
Proceed with Endoscopy as advised by your doctor.
Don't concentrate too much on the Abdomen.Anxiety can also lead to Acidity and also Irritable bowel syndrome.Be relaxed.

Ok, doctor.
I forgot to tell you where the abdominal pain is. I have attached an image to show where the pain is. When pressed on that region (the one in blue) the pain shoots to regions marked in (green). It feels like soreness underneath in the region. The pain in the region is green is not all over but anywhere within the region.

Off lately, the burping starts before dinner and goes on till I go to bed , this is also accompanied by lot of flatulence (which kind of smells).

Please do let me know if the pain could be due to acid issues or something else?

You can find the image in the reports section, this is a new image.

Detailed Answer:
Hi
I saw the new picture.
Oesophagus ends in the blue mark and joins Stomach. In the green area, stomach ends and the gut continues a Duodenum.
Any ulcer in the stomach/Duodenum will cause pain in these areas.
As it is in the upper part of the Abdomen, it could be due to Acidity.
Please come back after Endoscopy,I will clarify further
