I Have Had A Few Pains In My Groin Area.

1. Heavy feeling in my testicles
2. My right testicle is dropping lower than left
3. Aching feeling in the area.
4. Swelling in the testicles.
My urologist evaluated me Friday. He did a physical exam. He said he did find any lumps, odd shapes or hard spots in my testicle or groin area. He did note the right ball is significantly lower. But he wasn’t concerned. He also noted he felt slight swelling.
His assessment was that it could be that i had a bacterial infection. That came and went (i had strep in November) and the infection is working out my system. He said it could also be inflammation as the swelling happens mostly after ejaculation.
Questions:
1. I was unable to provide a urine sample on Friday as didn’t have to go. Should i request a urinalysis?
2. I had a CBC blood test. All normal. Should i request another type of blood test? If so, what?
3. Is an ultrasound needed?
4. What else could it be?
My doctor said to give it another 2 weeks to get out of my system and contact me if it’s not cleared up. He was opposed to another antibiotic due to the fact i just finished amoxicillin for strep last week.

Urinalysis and ultrasound
Detailed Answer:
Hi Darahnw1987,
1. You should definitely have a urinalysis to check for infection.
2. No need for other blood tests.
3. An ultrasound is a very good idea.
4. From your description it could be epididymitis, hydroxide, varicocele or scrotal herniation.
In my view, recent use of amoxicillin should not necessarily rule out the possibility of ongoing testicular infection, as bacteria involved in such infections typically do not respond to penicillin.
Hope this helps. Please let me know if you have further questions.

1. What is hydroxide?
2. Is scrotum hernia something that requires surgery? (I did have a sports a non surgery sports hernia isssue in 2014)
3. Will the urinalysis test for epididymitis?
4. Additional insight - My wife had a suspected UTI a month ago as well. We did have unprotected sex 4 days later. If that helps.
These questions should help until i get my results from the urinalysis and then the ultrasound if needed.

Hi again
Detailed Answer:
Hi again Darahnw1987,
1. Sorry, I meant to spell hydrocele, which is a kind of cyst in the scrotum, completely benign.
2. A scrotal hernia does not necessarily require surgery, only if you have symptoms.
3. A urinalysis can help determine if there is a urinary infection, e.g. e. coli that has migrated to the epididymides and caused infection. A urethral swap for gonorrhea and chlamydia is also advisable.
4. Thank you for that information. It is probably not related, but could indicate an infectious etiology to your symptoms.
Good idea to get the urinalysis and an ultrasound, that will almost definitely provide the diagnosis.
Hope this helps. Please do not hesitate to write again if you need clarification or have other questions.

My urologist did the physical exam cough, physical exam on testicles and abdomen. I had CBC blood work and chest x Ray. All normal. G.I check or throat and neck came back fine.
My urologist wants to hold off on the ultrasound until after Xmas. Is this urgent and need it now?
He wants to wait to see if inflammation and old infections are gone.
Concerned because of the pain being pretty constant (3 on a scale of 1-10). And swelling that comes and goes.
With some tests done now which of the guesses you mentioned above are still possible? What else could it be/ what should i look for?

Probably hydrocele or varicocele
Detailed Answer:
Hi again Darahnw1987,
You shouldn't worry, if you do not have fever or experience worsening pain, you're OK. With a normal urinalysis it could still be a varicocele or hydrocele; both completely benign, but annoying.
Do you have fever and can you keep the pain on a tolerable level with over the counter analgesics?

I have an anti inflammatory for costochondritis that my PCP recommended and my urologist said could work here. I guess I’ll start there. Thanks for all your help. Have a happy holiday
