Posted on Wed, 18 Mar 2020
Question: I have had a tension headache with some light headedness for 10 days.
I went to a l express clinic and was given nasal spray:

1. he said my ears look like they need to pop
2. He said it was nasal congestion there
3. The congestion between the two could cause a headache.

I’ve been taking the nasal spray for 2 days without any relief. What is the next step? How long until i should get additional evaluation?

The headache is a 4 out of 10 and it doesn’t get worse. It sometimes gets better

Detailed Answer:
Hii.

Are you having any allergic nose symptoms like sneezing, watering from nose, nasal obstruction? Are you allergic to dust, cold? It seems like you may have allergic rhinitis which is allergic inflammation of nose if you are having above symptoms. Do a x ray paranasal sinus and review. If it is possible you can undergo a nasal endoscopy and see the exact picture of the nose. Continue the nasal spray as it will reduce inflammation but it will take at least 1 week to have some effect. In the meanwhile start saline nasal spray to clear the mucus. Take steam inhalation as it will help in decongestion of the nose. So your next step is continue the spray, do a x ray sinus and wait for another 2 week. If headache and congestion didn't get resolve within 2 week get a nasal endoscopy done. Continue the fluticasone spray prescribed your doctor. If you are having light headedness check your blood pressure and sugar.

Detailed Answer:
Hii. It can be allergic rhinitis or migraine. Continue the spray for now. Take steam inhalation. Do a x ray sinus. It can exist for more than 2-3 week or reappear intermittently if it is migraine. You can take paracetamol for headache. Ask your ent if it seems to be like migraine and if need to consult a neurologist.
