I Have Had Problems With Anal Fissures Postpartum. I Went

Question: I have had problems with anal fissures postpartum. I went to the doctor 2.5 months ago with my first fissure which hurt a lot and bled a lot. It healed in a week but reopened 3 weeks after. I went back to the doc he said the fissure hadn’t changed. It stopped bleeding and hurting in a week then 5 weeks later I bled a little. I went to another doctor a week after and they said it was gone and the anus was jus raw due to hydrocortisone treatment (which I had stopped 3 weeks prior. I have used it for 5 weeks). She told me to put aquaphor on.
My treatment was that I had done hydrocortisone cream for 5 weeks, sitz bath, laxatives. I stopped all this and put aquaphor on
Today, a few days after the last doctor appointment, I felt a slight nerve pain with bowel movement and a little bit of blood on the tissue. (Attached) I’m not on laxatives but I didn’t think the movement was that hard.
What should I do? Is it possible that the new doctor just missed where the fissure is?

Brief Answer:
A reoccurrence is possible.
Detailed Answer:
Hello dear, welcome to Ask a doctor's service.
I read your query and here is my advice.
I see a very small amount of blood in the photo you attached.
I think that fissure was getting better and as you stopped the treatment a reoccurrence is happening.
I think you should continue treatment for another week and then get evaluated.
It may need more time to get healed.
Hope I have answered the question.
Let me know if I can assist you further.

Thanks for this. What should I do to continue treatment. I don’t have time for sitz bath is it ok if I don’t do that? I was taking hydrocortisone for too long and my bottom got a rash. Can I just have laxatives and fiber?
Also, as I said this is the 4th time it reopened in 3 months. Am I at risk for surgery or is it ok and I can just try to treat at home?

Brief Answer:
Surgery may be the last option.
Detailed Answer:
Hello dear and thank you for asking again.
I think you can do warm baths especially after going in toilet.
Try medical treatment before surgical treatment.
Use laxatives and fibers to soften stools.
You can use nitroglycerin externally to increase the blood flow and relax anal sphincter to help healing.
