Cancer of gall bladder can also present like this but as you said, with a history of 5 years this seems unlikely.
I'd suggest getting an ultrasound of the abdomen done to look for the cause.

I had ultrasound and MRI last month both were normal. CT scan in February. Have had gallbladder removed some time ago. I worry about pancreatic cancer.
