Posted on Tue, 10 Sep 2019
Question: I have high iron levels in my blood,

My liver function tests show high in two different parts one being Gamma and the other art (or something similar)
I have pain over my right side sometimes,
That flares up and goes away again,

Last week I have light colour stools but now normal.

The last time it flared up I felt very unwell which was a month ago this time I’m ok just a bit sore.

I haven’t lost weight or wouldn’t say the problem is getting worse

I have aches all over my body but this is been happening for a while

Can I get some advice?

Brief Answer:
further work up required

Detailed Answer:
Hi and welcome.
In such young person, these findings should be extensively evaluated, especially considering elevated liver enzymes.
In most cases this is not serious issues such as mononucleosis, fatty liver ,alcoholic liver damage or biliary stones. But I suggest to do some tests to rule out hemochromatosis, viral hepatitis and autoimmune hepatitis. To do so, you should start with liver ultrasound, check antibodies for hepatitis and fibromialgia, viral markers and check feritine levels in blood.

If this is fine, then liver biopsy may be considered.
In every case, before the right diagnosis is established, reduce alcohol intake, avoid hepatotoxic medications, regulate body mass and have healthy diet.
FOr pain, it would be best to take paracetamol.
WIsh you good health. Feel free to contact me for further questions.

Brief Answer:
I agree

Detailed Answer:
Hi. Yes I agree, this is most probably fatty liver as I mentioned, which can be regulated with dietary changes. But it would be good to have liver results every few months to see if GGT and SGOT will increase continously and then further work up can be planned.

Brief Answer:
Yes

Detailed Answer:
Yes, it may be related to high iron values as well. However, if these values increase continuesly you should do work up that I have mentioned. The pain you describe is suggestive of fatty liver nitnalso may be biliary stones or gastritis. I suggest to take ranitidin 150mg when pain is felt over the stomach and follow hepatic diet.
