Question: I have intra muscular fibroids and endometriosis and normally very painful periods. Have gone back on the pill after failing to conceieve for 6 years. Pill is to ease period pain.
Have period and today had very painful cramps and nausea and then passed a large white fleshy triangular lump with a blood clot attaches. Have never had this before and concerned

So, it is completely normal to have such findings and there is nothing to worry about. On the other hand, if you are bleeding profusely, you may need to have a CBC to make sure you are not developing anemia due to the heavy bleeding.

Oral contraception may help with the heavy bleeding and the pain with the menstruation. You may also take NSAIDs such as Ibuprofen or Naproxen. These need to be taken only during the menstruation and may help both with the pain and the amount of the bleeding. They have to be taken after food to protect your stomach.
Tranexamic acid can be used to reduce the amount of bleeding as well- you may need to talk to your doctor about it.

Removing the fibroids may be considered if they are the likely cause of your heavy bleeding. In some cases, IUD with progesterone (intrauterine device), such as Mirena, can be an alternative treatment to heavy bleeding.
