I Have Lichen Plantus On The Left Side Head Of My Penis
Question: I was recently told I have lichen plantus on the left side head of my penis. It started out as the tipical angular raised circle and not painful. 1 week goes by and it’s sorta red kind feel tingle pain and got checked. I was given clobetasol to use for 2 weeks I’m Currently on day 9. The clobetasol is working as it has gone down but another spot on the side of it is not a circle but super small wrinkle that is right on the edge of the penis head. I am cercumsized but the area is super sensitive and wax wanes in pain? Why do I feel pain? I though lichen plantus wasn’t really painful? Is this different or signs of developing something worst? I don’t see ulcering just small fishers that look like plantus. Is these fishers causing the pain? I seen a dermo and they gave me mid grade ointments because she thought clobetasol
Was too powerful but the mid grade ointment burned when I put them on. I went back to the clobetasol. What can I do to deal With the annoyance sensitivity and pain? I have to work I switched to cotton under ware baggy to help it not touch anything. I’m scared. I see a different dermo that focuses on Lichen plantus and the other variants. Why is it painful? The area looks so good but the pain is worst then ever and I don’t think it’s the cream

Brief Answer:
Stop clobetasol
Detailed Answer:
Hello
Thanks for the query and photographs.
Presently no lesions of lichen planus are visible on your penis.
The wrinkling of glans is due to use of steroid.
steong steroid creams such as clobetasol cause thinning of skin and make it sensitive.
Steroids give temporary relief but actually cause a lot of damage.
You need to stop using clobetasol immediately.
Apply a thick coat of petroleum jelly three times a day.
Take capsule vitamin E 400 mg daily for a month.
This should help you.
Just relax you have no disease and burning sensation is not due to any underlying diaease but due to over use of super potent steroid.
Let me know if you have any other doubts.
Thank you.

I sent another photo. In the yellow circle I drew it’s hard to see but on my end is red and in between the wrinkles it looks like it’s forming deeper fishers and that’s where my pain is in this spot and in general near the original circle of lichen that started a few weeks ago
I will wash it off genteelly and add petroleum jelly.
The dermatologist gave me
EUCRISA 2 % ointment
Generic name: crisaborole
Learn more
Apply to the affected area(s) ofpenis twice daily after one week of triamcinolone. if burning occurs this can be mixed with triamcinolone for
PRAMOSONE 1-1 % lotion
Generic name: pramoxine-hydrocortisone
Learn more
Apply to affected areas of rash as needed for irritation and pain
triamcinolone acetonide 0.1 % ointment
Commonly known as: KENALOG
Learn more
Apply to affected areas of penis twice a day for one week and then switch to nonsteroidal agent
All these burn and hurt which is why I don’t use them so still stick with petroleum jelly and wash this off gently?

Hello
Thanks for the follow up and photograph.
I can see a small violaceous lesion which you have encircled.
This might be a single lesion of lichen planus however it is insignificant and active treatment is not required as of now.
Your glans mucosa has become very thin and hence sensitive.
The creams prescribed to you are for treating Lichen planus but in present condition if you apply any of these creams you will have burning sensation, hence it is better to avoid them.
Just apply petroleum jelly and if you have any discomfort with that then just apply coconut oil.
Hope this helps.
Let me know if you have any other doubts.
Thank you.

This hurts badly no matter what I put on it and I have to work tomorrow how can I endure this? Anytime anything touches it or the head it hurts I’m scared from this like is those lessons the start of erosive lichen planus? It’s been 3 weeks since I noticed the first small ring and now just today noticing the lesion.
What’s the progression rate for the erosive kind? I hope it’s not

Hello
Thanks for the follow up.
It's not erosive lichen planus.
Such variety is usually seen in oral cavity and the lesion generally starts with a water filled bullae or as a erosion.
The lesion on your glans is a papule which is not going to erode.
Just relax you first need to regenerate your mucosa of glans.
It is definitely going to take time. Usually it takes around 2 months for substantial benifit.
Just apply petroleum jelly.
If it is too sensitive to touch and you have to go for work then you can try putting on a condom.
I usually suggest this for my patients and many of them have reported good relief.
Let me know if you have any other doubts.
Thank you.

These lesions hurt soo bad. Pain scale 10. I have been up all night tried antihistamine to go to sleep but can’t bwcaue of pain. Today is 3 weeks week one angular raised circle and week 2 starting to hurt and red both ring and inside the ring so whole papual was red. Clobetasol made it go down and worked with inflammation. Some parts on ring I see is red but the worst is the little pucks or lesions that have formed. There is the one I showed you then 2 other small Ones right at the fold over of the head. You think these lesions were caused by the clobetasol or do you think the lesions are the course the lichen planus is taking? I should go on pretnizone to make sure this gets stopped and nodes to tune into something crazy
Also would lidocaine cream or ointment work for pain? Or do you think it would sting and not be effective?
Brief Answer:
Lidocaine can be used
Detailed Answer:
Hello
Thanks for the follow up
The lesion within ring looks like lichen planus.
Clobetasol has caused wrinkling and furrowing and increased sensitivity of glans penis.
Prednisolone is again steroid so better to avoid it.
Lidocaine cream can be used. You can try it.
Let me know if you have any other doubts.
