I Have Lower Extremity Edema Bilat/ Tachycardia 100-110 At Rest;
Question: I have lower extremity edema bilat/ tachycardia 100-110 at rest; >130 with minimal activity (brushing teeth, rinsing a dish)/ severe shortness of breath with that minimal exertion. When I sit down there is NO palpable pulse in either carotid even though it is pounding in my chest/ if I don't sit down I get lightheaded near passing out. EKG + echo were WNL two weeks ago. Postural hypotension was documented. I was told to be sure I'm hydrated + add salt to diet. Is there any reason to seek another opinion. This tachy has been present > 6 months and all above are getting worse. I feel something else is wrong (I am 72 yrs old + an RN,BSN x 44 yrs.) Thanks. I am 5ft 7, 156#; nonsmoker, no alcohol, sedentary r/t spinal + major joint osteo with grade 4 damage both knees but can't do surgery. Asthma x 70 yrs but lungs clear + controlled with Singulair. No inhaler needed x 3 yrs. On Morphine 15mg TID, Gabapentin 800mg QID, Mobic 7.5th QD, Tramadol 50mg PRN.

Also I would like to know have you got your haemoglobin levels checked as low haemoglobin can also be the cause of unexplained tachycardia.

I do not have access to the EKG report or tracing. The results of lab work and the echocardiagram will be attached. Those were all tested in January, 2019. Thank you.

All your reports as a whole or over all within normal limits. However as you are giving a history of present copy along with feeble no palpable Carotid artery pulsations so it’s advisable that you should get one carotid artery ultrasound done to rule out Any significant underlying carotid artery stenosis.
Regarding postural hypotension you need to follow the advice given by your treating doctor. Also you can discuss with him about carotid artery Doppler.
