Detailed Answer:
Hi.
Thanks for your query, albeit short and specific.
Noted all the history and understood the concerns.
Please give additional information for me to assist you better:
Please watch for any red rash from mid-back to the area involved, may be present now or sometimes develops within few days.
Is the pain burning type? Or any other type/character like throbbing, cutting or so?
Any history of direct or indirect trauma to back recently or in past?
Any other symptoms like not feeling well, body ache, fever or so?

Awaiting for your detailed feedback point-by-point.

There is no rash or burning and no recent trauma. I usually have back aches due to the arthritis. But it is usually low or mid back. This is the upper back and what concerns me is the fact that it goes to the area under my breast the pain is circular.

Brief Answer:
As detailed below.

Detailed Answer:
The spread of the pain from upper back to the area of under the breast, in a circular fashion classically occurs when the pain is of nerve origin.
Back ache due to arthritis is always remain local and does not spread.
By meaning of a red rash I intent to see for Herpes Zoster or Shingles as is called.
Another reason can be nerve compression at the upper thoracic spine level wherein the cause can be found out by Spine MRI.
Any condition of the ribs that involve nerve can mimic the presentation but is rare and usually the pain is more localized, referred pain is rare to occur.

Please watch for this, consult your Primary Physician to look for the above mentioned signs, have clinical check-up and get MRI of thoracic spine if he/she thinks so.
This will give a probable diagnosis and timely treatment. This is so because the anti-viral medicines work well if Shingles is diagnosed as early as possible.
If there is any nerve compression then the treatment will be as per the cause found.
In addition you get prescription based medications.

I hope this answer helps you further for early diagnosis and proper management.
