Question: I have posted several times with this issue and although I fell it is improving and is not there constantly, I would like to use this months membership to get one more further opinion and advivce I have already got really helpful answers from 2 doctors but want to get a second opinion from a OBGN I had an episode about 4years ago of severe burning in the vulva. After many trips to different GPs they decided that it was nerve pain and over sensitive nerves. I was prescribed 20mg amytripteline and anesthetic gel. After couple of months it went totally but I was advised o keep taking the amytripteline and put the dose down to 10mg. I looked on the internet for what this may be and saw a whole string of depressing sites about vulvodynia and chronic pain. I was totally without any pain for 4 yrs until now. it is slightly better today and I am wondering if this could be the above and if so I am in some sort of flare. do you know anything about this ? any advise would be realyy helpful. When I wrote this the burning was quite bad , but seems to have gone now and just left a slight itch and pins and needles. Could this go away again for a long period and just flare up sometimes?

Your symptoms seem to be related to vulvodynia due to a nerve compression.

I suggest you to take over the counter medication to treat the pain such as Naproxen 500 mg two times a day.

The preferred medication to treat neuralgia is Gabapentin which is a prescribed medication.

You should consult your doctor about this treatment before taking it.

Some other advice that I can give to you are:

-Wear loose-fitting pants

-Heating pad can reduce pain

-Avoid foods that make urine more irritating such as beans or chocolate

-Do regular urinalysis to treat any possible infection

-Avoid using unscented shampoos

The symptoms of neuralgia based on daily practice usually flare up sometimes.

It is very important to be maintained under control with proper treatment and regular check ups.

Thank you for your advice. I see now from all the opinions that most of the advise is the same, so it has confirmed I am doing all the correct things. I was completely sysmpton free for almost 4 years and to be honest since the flare up 3 weeks ago it all seems to have calmed down enough to not interfere with daily life. Its all the terrible stories you see on the interent that bring you down and start the cycle of depression. I see now that this is something that can come and go through life. Have you experiences many patients with this? I your opinion is this a usually treated successfully . Thank you

I have come across many cases of neuralgia and also vulvodynia in daily practice.

They are all maintained under control through medication.

In the case of flare-ups, we can also give anti-inflammatory medications such as Ibuprofen 400 mg three times a day.

Another medication that can be taken in case of flare-ups is vitamin B12, which is a very good support for the nervous system.

I also recommend avoiding stress and do regular checkups.

Gosh, yesterday I had an urge to itch badly whilst at work. I usually have an ointment but it had run out. I then put Vagisil on and my whole vulva area swelled to tice the size. It has gone down this morning leaving a very slight itch. I am waiting for an appointment to see the Gyno at the hospital but have been tod it will be around 9 -12 weeks. I am seeing the GP tommoroow as I want he to see how much damage I had done down there and what to do if that occurs again.


Can you please let me know how succesfull the treatment is for this, in the patients you have treatd with vulvodynia, and if some become totally symptom free? I am so glad I don't have these symptoms every day, but each time it comes back I worry about it going away again.

I am really sorry to hear that you had this episode of flare up symptoms.

It looks like the symptoms may be related to a yeast infection or vaginosis which cause vulvodynia.

I suggest you to do a swab test to confirm the cause of the symptoms.

I also suggest to use Cetirizine 10 mg daily as I mentioned above.

Generally the symptoms get better in time with treatment.

As I mentioned above, you may have flare up symptoms, so I can not say that you will get symptoms free.

I suggest you to take the supplements that I mentioned above and do regular checkups to maintain it under control.

Thank you, I saw the Gp yesterday, it was pretty in helpful she suggests raiding amitripilyne to 40 mg and prescribed a betnovste cream for the itching. I feel the itch is worse st night but my GP had never heard of this so she is not too good with any advice . Do you have any of your Vulvodynia patients return on a regular basis and have you had any patients for whom it has gone away completely. Just worried I’m not going to be the same again thank you

Betonavate cream is a very good alternative to treat inflammation and itching so I suggest you to use it.

I don't have any patient who is completely cured from vulvodynia because it is a chronic medical condition.

But, most of the patients through regular medication to maintain under control the symptoms and live a normal life.

I suggest you to avoid stress, which can trigger the itching even more.

I also suggest to follow all the advice that I give above to avoid potential trigger factors.
