Citrobacter koseri is a normal habitat of water,soil & digestive tract of animals as well as of human digestive tract.
However,it generally causes clinical symptoms in neonates or immuno-compromised adults. In neonates these generally cause brain infection like meningitis & has a mortality rate in the range of 30-70%. In adults the species can be isolated in 1-2% cases of all UTI patients.It can have upward transmission in the urinary tract to kidney. Similarly from the colon it may go upward upto duedinum causing duedonitis, as in your case.
Usually it first form the early massive necrosis at the local site of infection,giving rise to cavity formations which may be filled with pus later on. Infection in the brain is a serious consequence of this infection,particularly in neonates.
The main treatment of it is by the sensitive antibiotics which may be carbopenem,cephalosporines, piperacillin with tazobactam, quinolines & doxycycline etc.
However,resistance to multiple antibiotic is becoming a problem,hence culture &sensitivity test should be a basis for antibiotic treatment. Recurrence is also a problem,therefore requiring periodic urine analysis & treatment if needed for some period at least.
