Well, it seems like a longterm problem where the pathology lies mostly in the speech generation and language organs. For further evaluation, you should get a thorough hearing profile (ENT check-up followed by Audiological tests, a lot of time, specific hearing impairments remain silent and undiagnosed since childhood).
Thereafter, Speech and language therapy can be planned under the SLPs (Speech and language pathology in Adults) therapists. It's routinely done as outpatient therapy and most of the problems can be overcome with this after a while.
Regarding panic, it seems to be a concern developed in the latter half of life (once the primary problem starts to get rectified, confidence will be regained over vocal command and you should be able to overcome it gradually). Also, do practice the speeches well, do homework before delivering on the given topics, things should be fine.
Please note, it's a longterm therapy for an ongoing problem, so don't rush into it. With time, routine practice, and follow-ups, things should be getting better. Starting daily exercise, yoga, meditation is generally proven to be another positive step, please consider it as well.
