Brief Answer:
fungal infection can be a cause

Detailed Answer:
HI, thanks for using healthcare magic

As you eluded to ,bacterial infections do not normally last such a long period, unless a person is colonized by certain bacteria such as stap aureus

Fungal or parasitic (leishmania) infections can persist for this length of time. Parasitic infections are not as common.

It can also occur as part of the symptoms in some autoimmune conditions such as lupus, sarcoidosis but in these cases, there would be additional symptoms not just the sore.

In very rare cases, the presence of a sore can be due to more serious causes but this is not likely since the lesion was present for 20 years.

It would be best to see an ENT specialist, a biopsy or swab of the area is needed to determine the exact cause.

IF this is not possible then you should visit your GP. A combination of antibiotic and antifungal may be given.
The antibiotic would be to cover stap aureus (in case this is the cause), though this is normally done after testing. ABX- clindamycin, trimethoprim/sulfamethoxazole, doxycycline
Anti fungals- itraconazole, terbinafine

I hope this helps, feel free to ask any other questions
