I Just Received The Results Of An MRI Where It

Question: I just received the results of an MRI where it said I have progressive Encephalomalacia. I had an MRI done XXXXXXX 2019 as a follow up to a benign brain tumor removal and this condition wasn't listed then. The tumor was quite large, described below. The tumor removal was done in two operations, one in November 2018 and the second in February 2019. The MRI from XXXXXXX 2019 was to establish a baseline to monitor it to see if it was going to grow back because they weren't able to get all of it. The measurements of the residual tumor given are .8cm x .3 cm and are consistent with what was in the XXXXXXX 2019 report so it's not growing. The exact wording in the new MRI report is ,"There is progressive encephalomalacia with mild progression in bilateral medial frontal T2 and FLAIR signal abnormality along the surgical margins."

I'm supposed to see them on Monday but am trying to prepare myself. Is this something that is usually lived with and it doesn't interfere with typical life activities or should I prepare for a continual degradation?

I'm not experiencing any of the more common symptoms I've read, sleepiness or loss of balance and motor skills. However, I didn't have any of that either with the brain tumor. The original tumor was described as follows in the 2018 MRI when it was discovered. "There is a large diffusely enhancing anterior parafalcine mass measuring approximately 8.0 x 5.5 x 7.5cm (AP x TR x CC) with associated hyperperfusion on ASL imaging and multiple internal as well as peripheral flow voids."

Brief Answer:
Some degree of malacia after surgery is acceptable.

Detailed Answer:
Hello and welcome to "Ask a Doctor " service.

I have read your query.

Some degree of brain malacia after surgical tumor removal is acceptable. In a certain point these changes stop from progressing.

Furthermore, you don't have any symptoms, so, you should not be worried much more than necessary.

The reason that malacia was not mentioned in the first MRI report is probably because it needs time to be evident.

In the meantime, it is necessary to control the risk factors of malacia progression such as high blood pressure, cigarette smoking, alcohol use,, etc.

In other words, you should follow a healthy lifestyle.

Since the tumor didn't enlarge, only follow up is sufficient in your case.

Hope you found the answer helpful.
